Dental Emergencies: Acute Infections and Pain Management Protocols
Dental emergencies present a significant challenge in clinical practice, requiring prompt diagnosis, effective pain management, and appropriate definitive or interim treatment. Acute odontogenic infections, in particular, can progress rapidly and may evolve into life-threatening conditions if not managed correctly. A systematic approach to triage and treatment is essential for ensuring patient safety and optimal outcomes.

Acute Apical Abscess and Cellulitis
An acute apical abscess is a localized collection of pus resulting from pulpal necrosis and bacterial invasion of periapical tissues. Patients typically present with severe, throbbing pain, tenderness to percussion, and thermal insensitivity of the involved tooth. If the infection remains confined within the alveolar bone, swelling may be minimal, but once the pus perforates the cortical plate, facial swelling becomes clinically evident.
The primary treatment for an acute apical abscess is drainage, either by accessing the pulp chamber and negotiating the root canal system to the apex or, when the abscess has localized, by incision and drainage through soft tissue. Antibiotic therapy alone is never a substitute for drainage, as the avascular nature of pus prevents antibiotics from reaching effective concentrations within the abscess cavity. However, systemic antibiotics are indicated when there are signs of systemic involvement such as fever, lymphadenopathy, or trismus, or when the infection has spread to fascial spaces.
Cellulitis represents a diffuse, spreading inflammation of soft tissues, often preceding abscess formation or developing when the virulence of microorganisms overwhelms host defenses. Cellulitis of odontogenic origin requires aggressive management with systemic antibiotics, close monitoring for airway compromise, and surgical drainage once the infection has localized. Involvement of deep fascial spaces, particularly the submandibular, sublingual, and lateral pharyngeal spaces, may progress to Ludwig's angina, a potentially fatal condition characterized by bilateral swelling, elevation of the floor of the mouth, and airway obstruction.
Pericoronitis and Alveolar Osteitis
Pericoronitis is an inflammatory condition affecting the soft tissues surrounding a partially erupted tooth, most commonly the mandibular third molar. The operculum, the flap of gingival tissue overlying the crown, becomes traumatized by the opposing tooth and harbors food debris and bacteria. Patients present with localized pain, swelling, trismus, and often a foul taste. Initial management includes gentle debridement and irrigation beneath the operculum with chlorhexidine or saline. Antibiotics are reserved for cases with significant swelling or systemic symptoms. Definitive treatment involves extraction of the involved tooth, though the acute infection should be controlled before surgical intervention if possible.
Alveolar osteitis, commonly known as dry socket, occurs when the blood clot within a post-extraction socket is lost or fails to form, exposing the underlying alveolar bone. This most commonly occurs two to four days after mandibular molar extraction. The hallmark symptom is severe, radiating pain that is disproportionately intense compared to normal post-extraction discomfort and unresponsive to over-the-counter analgesics. Risk factors include traumatic extraction, smoking, oral contraceptive use, and poor oral hygiene. Treatment focuses on symptomatic relief through gentle irrigation of the socket and placement of medicated dressings containing eugenol or other obtundent agents. The condition is self-limiting, typically resolving within seven to ten days as granulation tissue forms.
Traumatic Dental Injuries
Dental trauma encompasses a spectrum of injuries from enamel infractions to complete avulsion. The management of avulsed permanent teeth is particularly time-sensitive. The periodontal ligament cells on the root surface begin to lose viability within minutes of extraoral dry time. Immediate replantation at the accident site offers the best prognosis. If this is not possible, the tooth should be stored in an appropriate medium such as Hank's Balanced Salt Solution, milk, or the patient's own saliva while the patient is transported for emergency dental care.
Upon arrival at the dental office, the tooth should be gently cleaned of gross debris, handled only by the crown, and replanted into the socket after verifying correct orientation. Flexible splinting for one to two weeks, systemic antibiotics, and tetanus prophylaxis evaluation are standard protocols. Root canal treatment is typically initiated within seven to ten days, as revascularization occurs only in teeth with open apices. The prognosis for avulsed teeth depends heavily on the extraoral dry time, the storage medium, and the timeliness of definitive endodontic treatment.
Antibiotic Stewardship in Dental Emergencies
Antimicrobial resistance is a global health crisis, and dentistry contributes to this problem through inappropriate antibiotic prescribing. In acute odontogenic infections, most cases can be managed with local measures alone. Antibiotics should be prescribed only when there are clear indications of systemic involvement, spreading infection, or immunocompromised patient status. When antibiotics are indicated, penicillins such as amoxicillin remain the first-line choice for odontogenic infections. Clindamycin is reserved for penicillin-allergic patients. The shortest effective course should be prescribed, typically five to seven days for acute infections, with clear instructions for follow-up if symptoms persist.
Effective management of dental emergencies requires a combination of sound clinical judgment, prompt definitive treatment, and judicious use of antimicrobial therapy. By adhering to evidence-based protocols and maintaining vigilance for signs of severe infection, dental practitioners can ensure patient safety while providing effective relief from acute dental pain and infection.
