Other causes of orofacial pain, including temporomandibular disorders, glossopharyngeal neuralgia, dental pathology, and pharyngeal tumors, should be ruled out before treatment begins.

Conservative and Surgical Treatment

Treatment begins with a trial of conservative measures in patients with mild symptoms. Non-steroidal anti-inflammatory drugs, local anesthetic infiltration, and steroid injections around the styloid process may provide temporary relief. Transpharyngeal injection can be both diagnostic and therapeutic, since substantial improvement after anesthesia supports the diagnosis.

When conservative care fails or symptoms are severe, surgical treatment is the definitive option. The elongated portion of the styloid process is shortened through either an intraoral or an extracervical approach.
