Bruxism: Etiology, Diagnosis, and Occlusal Splint Therapy

Bruxism, the repetitive jaw muscle activity characterized by clenching or grinding of the teeth, affects a substantial portion of the population. Current international consensus defines bruxism as a behavior with two distinct circadian manifestations: sleep bruxism and awake bruxism. These are considered distinct entities with different etiologies, pathophysiological mechanisms, and management strategies, though they may coexist in the same patient.

Etiology and Risk Factors

The etiology of bruxism has evolved from a simplistic occlusal model to a multifactorial understanding involving central nervous system regulation, genetic predisposition, and psychosocial factors. Sleep bruxism is now classified as a sleep-related movement disorder, characterized by rhythmic masticatory muscle activity (RMMA) during sleep. It is not a parasomnia, though it may be associated with other sleep disorders such as obstructive sleep apnea, periodic limb movement disorder, and rapid eye movement behavior disorder.

Awake bruxism, in contrast, is primarily associated with psychosocial factors. Stress, anxiety, and emotional tension are consistently identified as major risk factors. Awake bruxism typically manifests as tooth clenching rather than grinding, often occurring during periods of concentration or emotional stress. Unlike sleep bruxism, which involves both phasic and tonic muscle activity, awake bruxism is predominantly tonic, with sustained muscle contraction over extended periods.

Additional risk factors include caffeine consumption, tobacco use, alcohol intake, and certain medications, particularly selective serotonin reuptake inhibitors and dopamine agonists. There is also evidence of a genetic component, with family studies suggesting a heritable predisposition to sleep bruxism.

Clinical Signs and Diagnosis

Clinical diagnosis of bruxism relies on a combination of patient history, clinical examination, and, in selected cases, instrumental assessment. Patients may report jaw muscle fatigue or soreness upon waking, temporal headaches, and awareness of grinding sounds reported by a sleep partner. Some patients are entirely unaware of their bruxism activity, presenting instead with consequences identified during routine dental examination.

The clinical signs of bruxism are predominantly dental and muscular. Tooth wear is the most visible manifestation, though its pattern differs from other forms of tooth surface loss. Bruxism-related wear typically presents as matching wear facets on opposing teeth, flat occlusal planes, and enamel chipping at incisal edges. In severe, long-standing cases, the wear can progress through enamel into dentin, causing loss of vertical dimension, dentin hypersensitivity, and compromised aesthetics. Masseter and temporalis muscle hypertrophy, particularly evident in chronic bruxers, results in a characteristic square jaw appearance. Indentations on the lateral borders of the tongue and linea alba on the buccal mucosa are soft tissue signs that, while not pathognomonic, support the diagnosis.

Polysomnography remains the gold standard for definitive diagnosis of sleep bruxism, recording RMMA through audio, video, and electromyographic channels. However, its cost and limited availability restrict its use to research settings and selected clinical cases. Portable electromyography devices for home sleep testing have been developed, though their diagnostic validity compared to laboratory polysomnography remains a subject of investigation.

Occlusal Splint Therapy

Occlusal splints, also known as night guards, oral appliances, or bite guards, are the most widely prescribed treatment for bruxism. While they do not eliminate bruxism activity—studies consistently show that RMMA episodes continue during splint use—they protect the dentition from the destructive effects of parafunctional forces and may reduce masticatory muscle activity through altered proprioceptive feedback.

Several splint designs are available, each with specific indications. The stabilization splint, or Michigan splint, is a hard acrylic appliance covering all maxillary teeth with a flat occlusal surface that provides even, simultaneous contacts with opposing mandibular teeth in centric relation. This design minimizes the risk of tooth movement and is the most versatile option for generalized bruxism management. The anterior bite plane, or anterior deprogrammer, discludes posterior teeth and is useful for cases where posterior muscle hyperactivity is the primary concern.

Soft splints, made from thermoplastic materials, are less expensive and more comfortable initially, but their long-term use is discouraged as they may increase clenching activity in some patients due to the elastic rebound of the material, functioning like a chew toy. Dual-laminated splints, with a hard exterior and soft interior, offer a compromise combining comfort with a stable occlusal surface.

Splint fabrication requires accurate impressions or digital scans, jaw relation records, and precise laboratory or in-office manufacturing. After delivery, occlusal adjustments are necessary to achieve the prescribed contact scheme. Regular follow-up appointments are essential to monitor splint wear, adjust occlusion as adaptation occurs, and assess for any adverse effects on the temporomandibular joints or tooth positions.

Adjunctive and Alternative Management Strategies

Comprehensive bruxism management extends beyond splint therapy. Behavioral interventions, including biofeedback and cognitive behavioral therapy, have demonstrated efficacy in reducing awake bruxism and may complement splint therapy for sleep bruxism. Stress management techniques, relaxation exercises, and sleep hygiene improvements address underlying etiological factors.

Pharmacological interventions, including muscle relaxants, botulinum toxin injections into the masseter and temporalis muscles, and low-dose tricyclic antidepressants, are reserved for severe, refractory cases. Botulinum toxin has shown particular promise in reducing bruxism-related muscle pain and hypertrophy, though its effects are temporary, lasting approximately three to six months, and repeat injections are required. Physical therapy modalities, including massage, ultrasound, and trigger point therapy, address the muscular component of bruxism.

Bruxism management is inherently long-term, as the condition is rarely permanently eliminated. A multidisciplinary approach that combines splint therapy for tooth protection, behavioral modification for addressing contributing factors, and periodic monitoring for complications represents the current best practice. With appropriate management, the dental and musculoskeletal consequences of bruxism can be effectively minimized, preserving both the dentition and the health of the masticatory system.
