Bruxism: Causes, Effects, and Modern Treatment Approaches

Introduction

Bruxism refers to the repetitive, involuntary grinding or clenching of teeth. It affects an estimated 8–31% of the general population and occurs in two distinct forms: awake bruxism (during waking hours) and sleep bruxism (during sleep). While occasional grinding may not cause harm, chronic bruxism can lead to significant dental damage, temporomandibular joint (TMJ) disorders, headaches, and reduced quality of life.

Etiology and Risk Factors

The pathophysiology of bruxism is multifactorial, involving central nervous system mechanisms, peripheral factors, and psychosocial influences.

Central Factors

  • Sleep microarousals: Sleep bruxism is classified as a sleep-related movement disorder, often triggered by transient arousals during non-REM sleep. Polysomnographic studies show rhythmic masticatory muscle activity (RMMA) occurs in close temporal association with cortical and autonomic activations.
  • Neurotransmitter dysregulation: Dopaminergic, serotonergic, and noradrenergic pathways in the central nervous system are implicated. Certain medications including SSRIs, SNRIs, and amphetamines can induce or exacerbate bruxism.

Psychosocial Factors

  • Stress and anxiety: Elevated psychological stress is consistently correlated with bruxism severity. Awake bruxism in particular is strongly associated with daily stress, concentration, and emotional tension.
  • Personality traits: Type A personality characteristics, high levels of neuroticism, and coping difficulties have been identified as predisposing factors.

Peripheral and Anatomical Factors

  • Malocclusion: While historically overemphasized, occlusal interferences may play a contributory rather than causative role.
  • Sleep-disordered breathing: There is a bidirectional relationship between sleep apnea and sleep bruxism. RMMA episodes often follow apnea-hypopnea events, potentially as a protective airway-opening mechanism.
  • Lifestyle factors: Excessive caffeine intake, alcohol consumption, smoking, and recreational drug use are established risk factors.

Clinical Manifestations and Diagnosis

Signs and Symptoms

  • Dental signs: Abnormal tooth wear (attrition), abfraction lesions, enamel fractures, cracked tooth syndrome, and restoration failure.
  • Muscular signs: Masseter and temporalis muscle hypertrophy, tenderness, and fatigue upon waking.
  • TMJ symptoms: Joint clicking, locking, limited mouth opening, and myofascial pain.
  • Other symptoms: Morning headaches (tension-type), sleep disruption, and tinnitus.

Diagnostic Approaches

  • Clinical examination: Systematic evaluation of tooth wear patterns, muscle palpation, and TMJ assessment using the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD).
  • Questionnaires: Self-report tools such as the Oral Behaviors Checklist (OBC) help quantify awake and sleep bruxism behaviors.
  • Polysomnography (PSG): The gold standard for definitive diagnosis of sleep bruxism, with audio-video recording and electromyographic (EMG) measurement of masticatory muscles.
  • Ambulatory EMG devices: Portable systems such as the BiteStrip and GrindCare provide home-based diagnostic and biofeedback capabilities, though accuracy varies compared to PSG.

Treatment Modalities

Occlusal Splint Therapy

Stabilization splints (Michigan-type) are the most common first-line treatment. They create a flat occlusal platform that reduces muscle activity, protects tooth structure from wear, and provides proprioceptive feedback to decrease parafunctional activity. Hard acrylic splints are preferred over soft ones, as soft splints may paradoxically increase clenching in some patients. Systematic reviews indicate splints effectively reduce RMMA episodes but show variable long-term efficacy.

Behavioral and Cognitive Approaches

  • Biofeedback: Devices that detect muscle activity and provide real-time feedback (vibration, sound, or electrical stimulation) help patients learn to reduce parafunctional behaviors. Contingent electrical stimulation has shown efficacy in reducing sleep bruxism episodes by up to 80%.
  • Cognitive behavioral therapy (CBT): Addressing underlying stress, anxiety, and maladaptive coping strategies. CBT combined with habit-reversal techniques shows promising results for awake bruxism.
  • Progressive muscle relaxation and mindfulness: Adjunctive techniques that reduce overall muscle tension and stress reactivity.

Pharmacological Interventions

  • Botulinum toxin type A (Botox): Intramuscular injections into the masseter and temporalis muscles reduce muscle force and RMMA frequency. Evidence supports efficacy in reducing pain and protecting teeth, though effects are temporary (3–6 months per cycle). Doses typically range from 25–50 U per masseter and 10–25 U per temporalis.
  • Muscle relaxants and anxiolytics: Clonazepam and cyclobenzaprine may provide short-term relief but carry risks of dependence and daytime sedation. Not recommended for long-term management.
  • Medication review: When bruxism is drug-induced (e.g., SSRIs), switching medications under medical supervision may resolve symptoms.

Physical Therapy

  • Masseter and temporalis muscle massage
  • Stretching exercises for masticatory and cervical muscles
  • Thermotherapy (moist heat or cold packs)
  • Transcutaneous electrical nerve stimulation (TENS)
  • Posture correction and cervical spine rehabilitation

Prognosis and Long-Term Management

Bruxism is typically a chronic condition that fluctuates in severity. Long-term management requires a multimodal approach combining occlusal protection, behavioral modification, stress management, and periodic reassessment. Splint wear should be monitored to prevent iatrogenic occlusal changes. Patients with concurrent sleep disorders should be referred for sleep medicine evaluation.

Bruxism is a complex, centrally mediated disorder with significant dental and muscular consequences. Contemporary management emphasizes protection of dental structures, reduction of muscle activity, and addressing underlying contributing factors. A personalized, evidence-based treatment plan yields the best long-term outcomes.
