Burning Mouth Syndrome: Etiology, Diagnosis, and Management Strategies

Burning mouth syndrome (BMS) is a chronic intraoral pain condition characterized by a persistent burning sensation in the absence of identifiable mucosal pathology. Affecting predominantly postmenopausal women, BMS presents a significant diagnostic and therapeutic challenge in clinical practice.

Definition and Clinical Presentation

Burning mouth syndrome is defined by the International Association for the Study of Pain as a chronic condition featuring a burning, scalding, or tingling sensation of the oral mucosa without clinically evident causative lesions. The International Headache Society further classifies BMS as an intraoral burning or dysesthetic sensation recurring daily for more than two hours per day over a period exceeding three months. The condition is broadly categorized into primary (idiopathic) and secondary forms. Primary BMS accounts for approximately 70 percent of cases and is considered a neuropathic pain disorder with no identifiable local or systemic cause. Secondary BMS arises from identifiable underlying conditions including nutritional deficiencies, endocrine disorders, xerostomia, oral candidiasis, allergic contact stomatitis, and gastroesophageal reflux disease.

The burning sensation typically affects the anterior two-thirds of the tongue, the hard palate, the labial mucosa, and the lower lip, though the distribution is often bilateral and symmetrical. Patients frequently report a progressive intensification of symptoms throughout the day, with relative absence upon waking and peak severity in the late afternoon and evening. Importantly, eating and drinking often provide temporary relief, a paradoxical feature that helps distinguish BMS from other orofacial pain conditions. Many patients also report accompanying subjective xerostomia and taste disturbances, particularly a persistent metallic or bitter taste.

Epidemiology and Risk Factors

The prevalence of BMS in the general population ranges from 0.7 to 4.6 percent, though estimates vary considerably due to inconsistent diagnostic criteria across studies. Postmenopausal women are disproportionately affected, with female-to-male ratios reported between 3:1 and 7:1, and peak incidence occurring between the fifth and seventh decades of life. The peri- and postmenopausal predilection suggests a potential role for hormonal factors, though the precise mechanisms remain incompletely understood. A systematic review by Kohorst and colleagues reported a pooled prevalence of 1.73 percent in population-based studies and 2.45 percent in clinic-based samples.

Several risk factors have been associated with BMS. Psychosocial factors including anxiety, depression, and personality traits such as catastrophizing and somatization are consistently reported. Studies demonstrate that over 50 percent of BMS patients meet criteria for a comorbid anxiety or depressive disorder. Additionally, specific genetic polymorphisms, including variants in genes encoding interleukin-6 and catechol-O-methyltransferase, have been implicated in susceptibility, supporting a biopsychosocial model of disease pathogenesis. Type 2 diabetes mellitus, thyroid dysfunction, and autoimmune connective tissue diseases also increase risk through secondary mechanisms involving metabolic neuropathy and mucosal changes.

Pathophysiology: Peripheral and Central Mechanisms

The pathophysiology of primary BMS is complex and involves both peripheral and central neuropathic mechanisms. Current evidence supports a model in which small fiber neuropathy of the trigeminal nerve triggers peripheral sensitization, followed by central sensitization that amplifies and perpetuates the pain signal. Histological studies of tongue biopsies from BMS patients have demonstrated a significant reduction in intraepithelial nerve fiber density, along with axonal degeneration and morphological changes consistent with small fiber neuropathy. Quantitative sensory testing further reveals altered thermal and mechanical thresholds in affected mucosal regions.

At the central level, neuroimaging studies using functional magnetic resonance imaging and positron emission tomography have identified altered activity in brain regions involved in pain processing, including the thalamus, anterior cingulate cortex, insula, and prefrontal cortex. The dopaminergic system also appears to play a crucial role. A landmark study by Jaaskelainen and colleagues demonstrated reduced striatal dopamine D2 receptor availability in BMS patients, a finding consistent with dysfunction in the nigrostriatal dopaminergic pathway. This dopaminergic hypofunction hypothesis provides a neurobiological basis for the anxiety and depression commonly observed in this population. Additionally, dysregulation of the hypothalamic-pituitary-adrenal axis and altered cortisol patterns are frequently reported, further linking BMS to stress-response systems.

Diagnostic Approach and Differential Diagnosis

Diagnosis of BMS requires a systematic clinical approach that combines a detailed history, thorough oral examination, and targeted investigations to exclude secondary causes. The history should document the onset, duration, daily pattern, location, and character of the burning sensation, along with exacerbating and relieving factors. A comprehensive medication review is essential, as several drug classes including angiotensin-converting enzyme inhibitors, antiretrovirals, and certain antidepressants can induce oral burning as a side effect. The clinical examination must rule out mucosal lesions, candidiasis, lichen planus, geographic tongue, and other visible pathology. Laboratory investigations should include a complete blood count, fasting blood glucose, serum ferritin, vitamin B12, folate, and zinc levels, as well as thyroid function tests.

The differential diagnosis includes oral candidiasis, which can be excluded by the absence of erythematous or pseudomembranous changes and a negative fungal culture. Allergic contact stomatitis to dental materials, particularly methyl methacrylate and nickel, should be considered and evaluated with patch testing when indicated. Xerostomia secondary to Sjogren syndrome, medication use, or head and neck radiotherapy can produce burning symptoms and requires objective measurement of salivary flow rates. Other conditions in the differential include atypical facial pain, glossopharyngeal neuralgia, oral lichen planus, and vitamin deficiency glossitis. The diagnosis of primary BMS is a diagnosis of exclusion, and clinicians should resist the temptation to label patients prematurely without adequate investigation.

Pharmacological Management

The pharmacological management of primary BMS remains challenging, with response rates varying considerably across treatment modalities. Clonazepam, a benzodiazepine with GABAergic activity, is the most widely studied agent. Topical clonazepam, administered as a 1 mg tablet dissolved in the mouth and held for three minutes before expectoration, has demonstrated efficacy in multiple randomized controlled trials, with response rates ranging from 50 to 70 percent. A systematic review by Cui and colleagues confirmed that topical clonazepam provides significant pain relief compared to placebo, with a favorable safety profile when used intermittently. Systemic clonazepam at low doses (0.25-0.5 mg at bedtime) may also be effective but carries risks of tolerance, dependence, and cognitive side effects.

Other pharmacological options include alpha-lipoic acid, a potent antioxidant that has shown mixed results across clinical trials. While some studies report significant improvement in BMS symptoms with oral alpha-lipoic acid at doses of 600-800 mg daily, a Cochrane review concluded that the evidence remains insufficient for a definitive recommendation. Capsaicin, applied topically as a 0.025 percent gel or rinse, desensitizes TRPV1 receptors and may provide relief in a subset of patients, though the initial burning sensation can limit tolerability. Low-dose tricyclic antidepressants such as amitriptyline (10-25 mg at bedtime) and selective serotonin reuptake inhibitors have been used with variable success, particularly in patients with significant comorbid anxiety or depression. Gabapentin and pregabalin, which modulate calcium channel activity and reduce neurotransmitter release, have shown benefit in case series but lack robust randomized evidence.

Non-Pharmacological and Psychological Approaches

Given the strong association between BMS and psychological factors, cognitive behavioral therapy (CBT) has emerged as an important treatment modality. CBT targets maladaptive pain-related cognitions, catastrophizing, and avoidance behaviors while teaching coping strategies and relaxation techniques. A randomized controlled trial by Komiyama and colleagues demonstrated that group CBT significantly reduced pain intensity and improved quality of life in BMS patients compared to a waitlist control, with effects maintained at six-month follow-up. The mechanism is thought to involve modulation of central pain processing pathways and improved emotional regulation.

Other non-pharmacological approaches include acupuncture, which has shown modest benefit in several small clinical trials, though methodological limitations preclude strong recommendations. Transcutaneous electrical nerve stimulation and low-level laser therapy have also been investigated, with some studies reporting symptomatic improvement. Patient education is a critical component of management; reassuring patients about the non-malignant and non-progressive nature of the condition can significantly reduce disease-related anxiety. A multidisciplinary approach that integrates pharmacological treatment with psychological support, nutritional assessment, and regular follow-up offers the best chance of meaningful symptom control.

Prognosis and Long-Term Management

The natural history of BMS is variable. Longitudinal studies indicate that approximately 50 to 60 percent of patients experience partial or complete spontaneous remission within five to seven years of symptom onset, though a substantial minority continue to experience persistent symptoms. Factors associated with poorer prognosis include longer duration of symptoms at presentation, higher baseline pain intensity, and the presence of comorbid psychiatric conditions. The management of BMS should be viewed as a chronic disease paradigm rather than an acute intervention model, with emphasis on setting realistic expectations, titrating therapies gradually, and providing ongoing support.

Regular follow-up at intervals of three to six months is recommended to assess treatment response, monitor for side effects, and adjust the therapeutic regimen as needed. The development of novel therapies targeting specific neuropathic mechanisms, including nerve growth factor inhibitors, cannabinoid receptor agonists, and botulinum toxin injections, represents an active area of research. Clinical trials investigating the efficacy of low-dose naltrexone for its anti-neuroinflammatory and analgesic properties are currently underway. As our understanding of the neurobiological underpinnings of BMS continues to evolve, more targeted and effective therapies are anticipated.
